- Full legal name (as appears on drivers licence), full address, and phone number.
- Copy of vehicle registration / ownership (doesn’t matter whose name its in)
- Exact mileage of vehicle.
- Clear pictures of the vehicle – 4 current pictures with the full car frame from all 4 angled corners, 1 wheels / tires, picture from front, picture from rear, interior seats, interior rear seats,interior radio climate control, odometer, VIN plate on door or engine.
- Any scratches, dents, issues with the car.
- Any reasons why you think the car is worth less than what the MTO says its worth.
- Any problems that will need to be repaired immediately, if applicable.
- How much you paid for the car or any receipt from seller showing the amount exchanged for the car.

- Clear pictures of the vehicle only if available prior to accident – front, rear, right side, left side, front seats, rear seats. If you don’t have any its okay too.
- Insurance Company Name, Policy Number, Date of Loss and Name & Phone Number of Person Handling the Claim
- How much you paid for the car or any receipt i.e.bill of sale if you have it.
- Copy of offer made to you by insurance company.
- Any receipts of any repairs you have made to car within last 1 year i..e tires,brakes,stereo etc. New or upgraded parts or aftermarket upgrades are helpful, if applicable.
- Why you think your car is worth more than the insurance company is offering.
- For Classic Car, any important modifications or notes.
- Any other pertinent information that may be useful.
